Liver Retrieval and Preservation

Liver retrieval and preservation are key steps in the transplantation pathway. Both steps are of fundamental importance in providing the best chance of a successful transplant. On arrival at the donor hospital, the retrieval team should introduce themselves to the local team, review all relevant documentation, and agree the retrieval process before starting surgery. Laparotomy and inspection of organs are essential before starting retrieval of organs. Perfusion techniques are developing and include normothermic regional perfusion, static cold storage, normothermic and hypothermic machine perfusion, and are likely to lead to improved organ viability. The main aim of liver preservation is to maintain the structural and functional integrity of the liver so as to promote liver function at reperfusion. The traumatic processes of brainstem or cardiac death, organ procurement, and preservation could have a detrimental impact on liver function post transplantation.
