SLET transplantation has become a recognised treatment for brittle Type 1 Diabetes Mellitus. Indeed, the short and mid-term results for this treatment are now equivalent to those achieved with vascularised pancreas transplants. It is therefore important for the transplant clinician to be familiar with the different aspects of this therapy. This chapter provides an overall summary of islet transplantation, including donor selection, islet isolation, recipient selection and work-up, the transplant itself, and the post-operative management and outcomes. © 2013 Blackwell Publishing Ltd.
