General medical research information
Thank you for considering taking part in research through the Oxford Radcliffe Biobank.
If you are receiving treatment within the Oxford University Hospitals NHS Foundation Trust (OUH), you may be asked by a doctor, nurse or a member of ORB staff whether you would like to help by donating samples to the biobank. This is the information sheet that would be discussed with you to seek your consent: Patient Information Sheet for general medical research
If you are receiving treatment within the Buckinghamshire Healthcare NHS Trust (Bucks Healthcare), you may be asked by a doctor, nurse or a member of ORB staff whether you would like to help by donating samples to the biobank. This is the information sheet that would be discussed with you to seek your consent: Patient Information sheet for general medical research (Bucks Healthcare)
If you have recently recovered from a period where you did not have capacity to consent and samples were taken after discussion with a Personal or Nominated consultee, you now have the opportunity to decide whether you want to continue to take part in research or not. This is the information sheet that would be discussed with you to seek your consent (same form for OUH and Bucks Healthcare patients): Patient Information Sheet for recovered capacity
